One using the supplied mDP to Digital port (4K at 60Mhz), the other using an HDMI to HDMI (4K at 30Mhz) and I've also got a small Wacom Cintiq using the DVi port on the graphics card.

Also many 4k TVs have only specific ports that can handle 4k/60 (on mine there's only 1 HDMI 2.0 port that can do 4k/60 in 4:2:0, the rest are limited to 4k/30), check your manual and make sure you are using the correct port. Tried resetting videocard drivers and restoring system to time before incident occurred. Did not help. P.S. on the same day my notebook stopped sending image to the monitor, however updating drivers resolved the issue.
